If you have a young person that you would like to refer to Get Psych'd please download and complete the below Referral Form.

We recommend completing the Referral Form in Adobe Acrobat Reader DC to ensure form functionality is optimised.

We encourage the Referrer to engage the young person and seek their consent to be referred to Get Psych'd.
